Ted Rosen, MD, professor of dermatology at Baylor College of Medicine in Houston, Texas, says this 61-year-old patient changed his approach to treating moderate-to-severe atopic dermatitis in adults.

More than a year ago, a woman came to Dr. Rosen for atopic dermatitis, which he describes as horrible.

My Neck Is On Fire!

“It wasn’t just her neck. It was on her elbow, her popliteal, her face… About the only place that wasn’t horrible and was just tender was her trunk, says Dr. Rosen.

A little history: the patient had allergic rhinitis and asthma throughout her life, but both of them resolved on their own. She had mild atopic dermatitis as a child, but it worsened with age, especially during adolescence.

The patient moved to Houston, where, according to Dr. Rosen, the heat, humidity and pollution only exacerbated her itchy skin — to the point where her ability to perform daily activities was impaired.

According to Dr. Rosen, she went through the usual medication routine. Topical steroids helped but did not clear up her skin. And because the atopic dermatitis involved the delicate skin of the face, neck and pits, Dr. Rosen says long-term use of heavy-duty steroids like clobetasol or topical halobetasol was not an option. Mild or moderate steroids didn’t have much of an effect, and antihistamines didn’t help at all.

“So we moved on to the next level of therapy: methotrexate, which is my backup systemic drug. She was intolerant of methotrexate, he says.

“Somewhere between three and five times, you start to worry about actually damaging your liver,” he says.

Next in line, Dr. Rosen tried ciclosporin, another effective drug for moderate to severe atopic dermatitis in adults. The patient had never had hypertension, but developed hypertension with significant systolic and diastolic elevations while receiving cyclosporine.

Dr. Rosen tried azathioprine (Imuran, Sebela Pharmaceuticals). She also did not tolerate this drug. The patient experienced gastrointestinal side effects including nausea, vomiting, and diarrhea to the point of dehydration.

“Then, not because I think it’s the best drug, but because I’m running out of options, I gave her mycophenolate mofetil (CellCept, Genentech) and it didn’t do anything, even after it worked.” he says.

She was not a good candidate for phototherapy because of her fair skin and history of actinic keratosis, basal cell carcinoma, and squamous cell carcinoma.

“Although phototherapy might have been helpful for her atopic dermatitis, it was really contraindicated because she had non-melanoma skin cancer. You don’t want to be doing phototherapy on someone with a long history of skin cancer,” says Dr. Rosen.

The drug that finally worked after all others had failed was dupilumab (Dupixent, Sanofi). The patient struggled to convince her insurance carrier to pay for a biologic, an IL 13 and IL 4 inhibitor approved by the FDA for the treatment of atopic dermatitis.

According to Dr. Rosen, the patient pictured after had only received three doses of dupilumab, or a month of treatment.

“It’s amazing,” says Dr. Rosen. “She said it was the best she had looked since she was 11. She literally said you gave me my life back.’

After a year of treatment with dupilumab, the patient has only occasional pruritus, which for the most part remains clear. The only side effect was eye irritation, which did not reach the level of conjunctivitis or blepharitis, according to Dr. Rosen.

Dermatologists may face hurdles to get insurers to approve expensive biologics like dupilumab, he says, but those hurdles are worth overcoming.

“Here’s the message (besides you giving me my life back): don’t wait. Move to the most effective drug that works for patients as soon as possible,” says Dr. Rosen. “If there is a patient objection to receiving biologics, try to address that and skip it. I must have gone overboard trying different things. I think it’s reasonable to try an agent like methotrexate or ciclosporin, and if that agent doesn’t work, see your insurance carrier for a biologic.”
